An internal audit reveals Lend Lease has recorded more than 60 serious safety breaches in a year which have put builders at risk of electrocution, fire, falls from height and falling objects.

The revelations come as tensions between the CFMEU and the company escalate.
Lend Lease chief executive Steve McCann recently called the union a ''disgrace'' and accused it of raising safety concerns on building sites as a distraction from the royal commission on union governance and corruption.
But an internal Lend Lease document shows the company has identified its own concerns with safety as part of a quarterly business review on March 28, when 72 active building sites across the country were ''placed on watch''.
The document, leaked to Fairfax Media, shows Lend Lease recorded 62 critical incidents including 30 relating to falling objects, including tools and scaffolding. It says the cases include failures to provide safety restraints and exclusion zones around areas where builders work from heights.
Six incidents ''relate to unprotected electric shock where we failed to adequately isolate and lock out the electricity supply''.
Another six incidents related to people falling through ceiling spaces, ladders and collapsed scaffolding. There have been 16 fire or smoulder incidents, mostly related to hot works such as welding.
CFMEU national construction secretary Dave Noonan said the company has acknowledged it had serious safety issues.
''The issue in relation to falling objects without proper exclusion zones, falls from height, risk of electrocution and collapse of temporary structures, tells us the company needs to lift its game when it comes to occupational health and safety,'' Mr Noonan said.
A spokeswoman for Lend Lease said the document is evidence of the company's ongoing commitment to safety. ''Safety is our number one priority,'' she said.
The document is part of the company's internal management processes and is ''not a legislative or regulatory requirement''.
''It is a self-imposed and proactive process we apply on some projects and in some business units to ensure safety incidents receive the appropriate response,'' the spokeswoman said.
''This includes resourcing and focus from senior management so that we can then implement preventative actions.
''This internal document is further evidence of our ongoing commitment to safety."
The company says the amount of time lost due to injury has decreased by a quarter in the past five years.
The CFMEU launched a national safety blitz of Lend Lease work sites across the country early this month after the Victorian Supreme Court issued it with a $1.25 million fine and criminal convictions for contempt of court after its blockade of a Grocon building site in Melbourne in 2012.
The company strongly rejected the CFMEU's claims it had compromised safety standards.
The union said Lend Lease was unable to confirm workers' safety for 2½ hours after a recent fire at its Barangaroo site in Sydney. Lend Lease did not dispute this claim.
Workers were also dismissed from a building site at the University of Technology, Sydney after the union reported breaches of occupational health and safety standards including failures in evacuation plans.
Lend Lease called police to the site, accusing the union of breaching its right of entry.
The company took the dispute with the CFMEU to the Fair Work Commission.
It is understood to be in conciliation talks with the union before possible arbitration.
